Family Medicine Student Rotations.

Dear Student or Resident:

After submitting a request for a rotation and receiving approval please review the following set of instructions and reminders pertaining to your rotation at United Hospital Center:

You will need to complete any required paperwork from your school and have it forwarded to the appropriate preceptor according to your school requirements. The student or student’s school is also responsible for providing any follow-up evaluation to the preceptor.

You will need to be assigned a 4-digit physician number. Please email Aimie Vincent at vincenta@uhcwv.org to get your number prior to beginning your rotation. You must do this before you schedule any of the sessions as listed below.

You will need to meet with Patsy Coll in Information Technology in order to be trained on the Physician Portal System that allows you access to patient records. You can call to schedule your session with her (15 to 20 minutes) by calling 624-2497. Preceptors do expect that you will not start with them until you have completed your orientation; feel free to schedule your session if possible the morning of your first day.

You will need to meet with Tina Matthey in Dictation to be oriented to the Dictation System. You will need to schedule your training session prior to beginning your rotation. Preceptors do expect that you will not start with them until you have completed your orientation; feel free to schedule your session if possible the morning of your first day. You can reach her to schedule your training session (approximately 15-20 minutes) by calling 624-2962.

On the morning of the first day of the rotation you must meet with Vicki Wagner at the Family Medicine Center on the UHC campus. You must fill out necessary documents before starting the rotation. Failure to do the required paperwork will result in dismissal from the UHC campus. You will be given paperwork to obtain an ID badge and sometimes beeper depending on the rotation. The beeper will require a $50.00 deposit.

If you have been approved for student housing at the house on Lee Ave, you will be required to review and sign the housing policy with Vicki Wagner on the first morning of your rotation. Your UHC ID badge will be activated to grant access to the student house. The access will remain active while you are listed on the master schedule as performing an approved rotation. Housing access will be deactivated 48 hours after the rotation ends.

All ID badges must be returned to Vicki Wagner at the end of the rotation. Lost or reissued badges will be replaced for a $20.00 fee. All beepers must be returned to Vicki Wagner at the end of the rotation. Non-returned beepers will result in forfeiture of the $50.00 deposit and jeopardize your opportunity to plan future rotations on the UHC campus.

Instructions and contact information for your assigned preceptor can be found in the Preceptor Instruction pdf.
